当参加者の表明および同意書はHINOKI YOGAが、ワークショップを安全かつ円滑に進めるための同意書です。
事前に内容を必ずお読みのうえ、ご同意下さい。また個人情報の取り扱い及び管理については、ご 記入いただいた参加者ご自身の個人情報の保護を重視し、その取扱については細心の注意を払いま す。同意書に頂いた個人情報は第三者に提供または提示いたしません。

（１）私（参加者）はスポーツやヨガに参加する事により、事故に遭い怪我を負う可能性があることを理解していま す。
（２）私は私が健康であり、ヨガのワークショップやHINOKI YOGA(以下｢当社｣という）でのクラスの参加に支障を きたすような疾病または身体的損傷はありません。
（３）私は当社でのクラスにおいて私の身体状況に関する医療判断を含む医療サービスが与えられる事が無い事を理 解しています。
（４）私は（１）～（３）の表明に基づき、自己責任と負担において、当社でのクラスに参加します。
（５）私は当社および当社が委託する運営者（以下｢運営者｣という）の責任に帰すべき事由による場合を除き、私が 当社のクラスに参加し、当社の設備を使用し、またその敷地内外でヨガやその他のスポーツやアクティビティにより 参加することより発生した、自己所有物の破損、損失・紛失や身の上の事故・怪我もしくは死亡に関し、当社および 運営者に対し、いかなる請求・要求・訴訟、その他一切の責任追及をしないことを同意いたします。
（６）私が当社またはその敷地内外において、私の責任に帰すべき事由により、当社または運営者あるいは第三者に 損害を与えた場合は、当社または運営者あるいは第三者に対し、その賠償の責任を負うことを同意致します。
（７）私は上記（１）乃至（６）の各条項を読み､その内容を理解した上で､本表明及び､同意書の末尾に署名いたし ます｡
AGREEMENT OF RELEASE AND WAIVER OF LIABILITY

Buyer is aware that participation in a sport or Yoga may result in accident or injury, and buyer assumes risk connected with the participation in a sport or Yoga and represents that Participant is in good health and suffers from no physical impairment which would limit their use of or participation in the, Yoga Workshop facilities and Yoga Center’s classes. （HINOKI YOGA)Buyer acknowledges that the Yoga Workshop and Yoga Center’s classes have not and will not render any medical services including medical diagnosis of paricipant’s physical condition buyer specificelly agree that the Yoga Workshop and Yoga Center’s classes it’s officers, employees, agents, guest instructors and facilitators should not be liable for any claim, demand, cause of action of any kind whatsoever for, or on account of death, personal injury, property damage or loss of any kind resulting form or related to Participant’s use of the Yoga Workshop or Yoga Center’s facilities(including facilities rented by the Yoga workshop for special classes or events)or participation in any sport Yoga or activity within or outside of these premises, and the Buyer agrees to hold the Yoga workshop and Yoga Center harmless from the same. I voluntarily agree to the terms and conditions stated above.
